Course Description:


This session explores how to effectively report, investigate and learn from incidents within care settings, with a focus on reducing future risk and improving practice.


Learning Outcomes:


  • Understand the purpose of incident reporting
  • Identify root causes using structured approaches
  • Apply learning to reduce recurrence
  • Improve documentation and accountability

Who is it for:


Care staff, team leaders and managers involved in incident reporting and investigation.

  • Module 3 – Legislation, Regulation & Inspection (3.3, 3.4, 3.5)
  • Module 6 – Positive Risk-Taking (6.4)
  • Module 10 – Continuous Improvement (10.2)
